Community Health Worker (Full-Time, On-site)

 

This position is primarily responsible to provide direct services to ACLAMO’S Community Health Connections/Conexiones de Salud Comunitaria clients. She/he will increase access to much needed primary care, behavioral and mental health, and educational services.  This position will be a liaison between clients, medical providers and other local service providers and community groups. The ideal candidate will be a strong written and verbal communicator, experienced in the health care industry, and proactive with solutions and adjustments. The Community Health Worker will be responsible for the following essential functions:

ROLE & RESPONSIBILITIES:

  • Completes an initial needs assessment and acts upon critical needs appropriately and immediately.
  • Develops a comprehensive client-driven case plan with both short-term and long-term goals identified. Completes a timeline and measures for each goal.
  • Monitors progress towards goals in regularly scheduled sessions.
  • Assist/Referred patients/clients in enrolling in health insurance and other health care services
  • Facilitate referrals to resources provided by partners and other agencies as needed.
  • Accompany patients/clients to critical health care or social service appointments offered by the medical providers and other community partners.
  • Facilitate opportunities for individuals to direct their own physical and behavioral wellness
  • Closely work on collaboration with the local area hospitals, county offices members, community partners and others.
  • Promote available service options and choices to the community and encourage participation
  • Participated on the development, focus groups and interviews of community members to identified their needs.
  • In partnership with other for profits and non profits partners, initiate and implement activities that will increase the  utilization of ACLAMO’s services
  • Work effectively in groups with staff in ACLAMO’s education, health promotion and wellness department.
  • Record accurate documentation of client files, outreach, referrals, program participation, and other relevant activities in a timely fashion to support monthly reports.
  • Enter data, submit applications and reports in a timely manner.
  • Attend weekly supervision, case reviews, trainings, team and other administrative meetings
  • Other duties as assigned.

 

QUALIFICATIONS AND EDUCATION REQUIREMENTS:

  • A bachelor’s degree from an accredited college or university in public health, health education or related field with 1 year of experience , OR
  • An associate degree in public health, health education, or related field and 2 years of working experience in the field.
  • Go through the CHW Certification Program and obtain certification.

Preferred Skills:

  • Bilingual (English/Spanish)
  • Proficient in Microsoft programs, eg. Outlook, Word, Excel, and PowerPoint.
  • Work with a variety of data management softwares using standardized methods and job-related equipment including ability to enter and track client’s data.
  • Comfortable working both independently and as part of a team.
  • Excellent written and verbal communication and interpersonal skills.
  • Must be detail oriented and possess strong organizational skills.
  • Work at establishing a base of trust with each resident or community member.
  • Accept residents and community members “where they are at” while working to assist them.
  • Be a passionate supporter of ACLAMO’s mission, vision and programs.
  • Observe confidentiality, privacy, and dignity of each person in the program.

    Special Notes:

    • Must have and maintain a valid drivers’ license, automobile insurance and daily access to an automobile and cell phone
    • Working days are Monday thru Fridays with occasional Saturdays and some nights and/or weekends.
    • This position requires visual and auditory acuity and the physical ability to ascending or descending ladders, stairs, ramps among others. Adjusting, lifting up, carrying up or moving objects up to 35 pounds in all directions. Standing or sitting for prolonged periods, walk and work in your feet for a minimun of 6 hours at a time.
    • Must obtain the following three clearances:

    Pennsylvania Child Abuse History Clearance;

    Pennsylvania State Police Criminal Record Check; and

    Federal Bureau of Investigation Criminal Background Check.
