Community HPP Health Navigator

Position type: Part-time/Hybrid 20 hours a week

Reports to: Director of Strategic Partnerships 

JOB OVERVIEW

The Community HPP Health Navigator is responsible for supporting ACLAMO’s Medicaid members. This role aims to eliminate barriers to health care and reduce access to primary care. The ideal candidate will be a highly organized and detail-oriented, strong and proactive communicator, experienced in case management and community outreach. Conscientious and aware of the needs of diverse populations, excellent leadership experience in similar roles is preferred. 

KEY RESPONSIBILITIES

Responsibilities Community Health Navigator:
  • Complete an average 25 “touches” per day to HPP Medicaid members via phone calls, text messaging, and email (as appropriate).
  • Provide one-on-one visits to members 2 days per week as needed to establish relationships, trust and accountability.
  • Expertly listen to and discuss with each member their health status, gaps in care, and perceived barriers to accessing care.
  • Clearly communicate information to members regarding remedies for stated gaps in care including HPP programs and resources.
  • Assist members in navigating and arranging SDoH resources and services available tothem through HPP, ACLAMO or other community agencies.
  • Guide members in networking with healthcare providers within their insurance network, scheduling appointments, and coordinating follow up.
  • Work closely with HPP to provide a warm handoff of members to the HPP Clinical Connections/Care Management Team as appropriate.
  • Record accurate documentation of member files, outreach, referrals, program participation, and other relevant activities in a timely fashion to support accurate monthly reports.
  • Enter data, submit applications, and submit reports in a timely manner.
  • Attend all ACLAMO staff and partners’ meetings, events and trainings as needed.

QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • Experience in Community outreach, Health Care Management or possess a bachelor’s or an associate degree from an accredited college in Health Management, Business Administration, Human Services, or a related field. Must be fluent in Spanish and English.  

WORK SCHEDULE

  • On-Site Work: Employee is required to work on-site for two days a week, with each day consisting of  5 hours.
  • Remote Work: Employee will work remotely for two days a week, with each day consisting of 5 hours. 
  • Duration: This schedule will be in effect all year round.  

 SPECIAL NOTES

  • Must have and maintain a valid driver’s license, automobile insurance, and daily access to an automobile and cell phone.
  • This position requires visual and auditory acuity and the physical ability to ascend or descend ladders, stairs, ramps, etc. Must be able to adjust, lift, carry, or move objects up to 35 pounds in all directions.  
  • Be able to stand or sit for prolonged periods, walk, and work on your feet for a minimum of 6 hours at a time  

Must obtain the following three clearances:

• Pennsylvania Child Abuse History Clearance;
• Pennsylvania State Police Criminal Record Check; and
• Federal Bureau of Investigation Criminal Background Check.
