The ideal candidate will be conscientious and aware of the needs of the specific population that ACLAMO serves and may have prior experience in a similar role. An ideal candidate will be highly organized, a strong and proactive communicator, and a leader with previous experience in case management.
- Hourly Salary: Competitive rate $22.60 -$24.50
- Work Type: Full time, hourly. Required to work up to 40 hours per week
- Work Schedule: Hybrid remote and on-site work, 4 days onsite 1 day remote. Monday through Friday, 8:30-4:30p. occasional nights and Saturdays.
- Location: Pottstown, PA.

Education Requirements
- Possess 3 years of experience in Community Outreach, Health Care Management, or possess a bachelor’s or an associate degree from an accredited college on Health Management, Business Administration, Human Services or a related field.
- Must be fluent in Spanish and English
Role and Responsibilities
Community Health Navigator (60%)
- Complete an average 35 “touches” per day to JHP Medicaid members via phone calls, text messaging, and email (as appropriate).
- Provide one-on-one visits to members 2 days per week as needed to establish relationships, trust and accountability.
- Expertly listen to and discuss with each member their health status, gaps in care, and perceived barriers to accessing care.
- Clearly communicate information to members regarding remedies for stated gaps in care including JHP programs and resources.
- Assist members in navigating and arranging SDoH resources and services available to them through JHP, ACLAMO or other community agencies.
Benefits Acquisition (40%)
Assist clients with enrolling in health insurance and other government benefits programs.
- Serve as a liaison between the client and the County Assistance Office and provide follow-up assistance as needed to ensure that the application includes all required supporting documents and is processed in a timely manner.
- Maintain updated information on health insurance, COMPASS, and SNAP policies and procedures.
- Work with the supervisor to maintain a file of all completed health insurance and SNAP applications, re-certifications, and supporting documents and a log with ID numbers of all applications and re-certifications submitted electronically via COMPASS.
Skills
- Proficient in Microsoft programs, eg, Outlook, Word, Excel, and PowerPoint.
- Work with a variety of data management platforms, using standardized methods and job-related equipment, including ability to enter and track client data.
- Comfortable working both independently and as part of a team.
- Excellent written and verbal communication and interpersonal skills.
- Must be detail oriented and possess strong organizational skills.
- Work at establishing a base of trust with each resident or community member.
Special Notes
- Must have and maintain a valid driver’s license, automobile insurance, and daily access to an automobile and cell phone.
- This position requires visual and auditory acuity and the physical ability to ascend or descend ladders, stairs, ramps, etc.
- Must be able to adjust, lift, carry, or move objects up to 35 pounds in all directions.
- Must be able to stand or sit for prolonged periods, walk, and work on your feet for a minimum of 6 hours at a time.
Must obtain the following three clearances:
- Pennsylvania Child Abuse History Clearance
- Pennsylvania State Police Criminal Record Check
- Federal Bureau of Investigation Criminal Background Check.
