Community Health & Food Access Supervisor

JOB OVERVIEW
The Community Health & Food Access Supervisor is primarily responsible for providing direct services to ACLAMO’S Community Health Connections/Conexiones de Salud Comunitaria clients. She/he will increase access to much needed primary care, behavioral health, food and educational services. This position will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes. The Community Health & Food Access Supervisor will be responsible for the following essential functions: .
ROLE AND RESPONSIBILITIES
- Identify the health and wellness needs of residents, potential clients, and community members.
- Complete an initial needs assessment and acts upon critical needs appropriately and immediately.
- Coach patients in effective management of their chronic health conditions and self-care.
- Develop a comprehensive client-driven case plan and instructions with both short-term and long-term goals identified.
- Monitor clients’ progress towards their service goals; document all interactions with clients via progress notes; complete monthly/annual reports and collect and enter data.
- Accompany patients/clients to critical health care or social service appointments offered by the medical providers and other community partners.
- Assist/refer patients/clients to enroll in health insurance, financial aid, and other health care services.
- Screen and enroll participants for food services, distribute/deliver food to participants, and provide low-threshold case management services to connect participants to relevant and needed services as needed.
- Support community outreach efforts including organizing food drives, tabling in the community, and maintaining and fostering new connections with local businesses, groceries, and farms.
- Assist staff to help recruit, train, and supervise volunteers in project identification and matching.
- Closely work on collaboration with the local area hospitals, county offices members, community partners and others.
- Participate on the development, focus groups and interviews of community members to identify their needs.
- Be willing to perform other duties as requested.
QUALIFICATIONS AND EDUCATION REQUIREMENTS
- A bachelor’s degree from an accredited college or university in public health, health education or related field with 5 years of experience , OR An associate degree in public health, health education, or related field and 8 years of working experience in the field.
- Go through the CHW Certification Program and obtain certification.
- Fluency in Spanish and English required.
PREFERRED SKILLS
- Able to prioritize and manage multiple projects simultaneously and follow through in a timely manner.
- Proficient in Microsoft Word, Outlook, Excel, and Power Point.
- Work with a variety of data management using standardized methods and job-related equipment including ability to enter and track client’s data.
- Comfortable working both independently and as part of a team.
- Excellent written and verbal communication, detail oriented, interpersonal and organizational skills and proactive with solutions and adjustments.
- Experienced in the health care or social services industry.

SPECIAL NOTES
- Must have and maintain a valid driver’s license, automobile insurance, and daily access to an automobile and cell phone.
- This position requires visual and auditory acuity and the physical ability to ascend or descend ladders, stairs, ramps, etc. Must be able to adjust, lift, carry, or move objects up to 35 pounds in all directions.
- Be able to stand or sit for prolonged periods, walk, and work on your feet for a minimum of 6 hours at a time
Must obtain the following three clearances:
• Pennsylvania Child Abuse History Clearance;
• Pennsylvania State Police Criminal Record Check; and
• Federal Bureau of Investigation Criminal Background Check.
