Senior Case Manager – (Full-Time, On-site)

The Senior Case Manager will provide direct case management services to community members. She/he will provide care efficiently, handle case assignments, document case plans, and review the progress of each case. The ideal candidate will be conscientious and aware of the needs of the specific population that ACLAMO serves and may have prior experience in a similar role. An ideal candidate will be higly organized, a strong and proactive case manager. The position includes the following essential functions:

ROLE AND RESPONSIBILITIES:

  • Interviews client, family and/or professional sources to determine clients’ needs for a variety of social services which may include social services, food insecurity, housing, employment, job search and basic skills remediation.
  • Develops a comprehensive cost effective service plan utilizing in-house sources, referrals, contracted providers and/or institutions.
  • Conducts on-going monitoring and assessment of service delivery for optimum quality and efficiency, and authorizes and recommends changes and adjustments as needed.
  • Maintains comprehensive case records, summaries, statistical and narrative reports and completes required forms.
  • Establishes effective working relationship with clients, family, community and professional sources.
  • Mediate and advocate on behalf of consumers to help them obtain and maintain permanent housing, health care, benefits, entitlements, and transportation
  • Provide case management services and case management support for adults, seniors, students and families including parents with children referred by the education program.
  • Link at-risk children and families to resources for health and education services.
  • Provide referrals to and advocate for special services
  • Guide families to health services and preventive health resources
  • Provide parenting classes and/or coordinated other life skills trainings for families and/or children.
  • Accompany clients to appointments, court hearings, school meetings, home visits as needed.
  • Enter and collect data to evaluate social services programs’ impact
  • Attend all ACLAMO staff meetings, field trips and trainings
  • Other duties as assigned.

QUALIFICATIONS AND EDUCATION REQUIREMENTS:

  • A Bachelor’s degree with 3 to 5 years of experience in human services. Degrees should be from an accredited college or university in human services, social services and related field of studies.

PREFERRED SKILLS:

  • Bilingual (English/Spanish)
  • Proficient in Microsoft programs, eg. Outlook, Word, Excel, and PowerPoint.
  • Work with a variety of data management using standardized methods and job-related equipment including ability to enter and track client’s data.
  • Comfortable working both independently and as part of a team
  • Excellent written and verbal communication and interpersonal skills.
  • Must be detail oriented and possess strong organizational skills
  • Work at establishing a base of trust with each resident or community member.
  • Accept residents and community members “where they are at” while working to assist them.

  • Observe confidentiality, privacy, and dignity of each person in the program.

Special Notes:

  • Must have and maintain a valid drivers’ license, automobile insurance and daily access to an automobile and cell phone.
  • This position requires visual and auditory acuity and the physical ability to ascending or descending ladders, stairs, ramps among others. Adjusting, lifting up, carrying up or moving objects up to 35 pounds in all directions. Standing or sitting for prolonged periods, walk and work in your feet for a minimum of 6 hours at a time.
  • Required working days are Monday thru Fridays and some night and weekends.
  • This position requires traveling to all ACLAMO offices.
  • Must obtain the following three clearances:

Pennsylvania Child Abuse History Clearance;
Pennsylvania State Police Criminal Record Check; and
Federal Bureau of Investigation Criminal Background Check.
