Bilingual Case Manager Coordinator – (Part-Time, On-site in Pottstown)

The Bilingual Case Manager Coordinator oversees the day to day operations in the Pottstown office including maintaining and developing new relationships with community members and partners. She/he will provide care efficiently, handle case assignments, document case plans, and review the progress of each case.  The ideal candidate will be conscientious and aware of the needs of the specific population that ACLAMO serves and may have prior experience in a similar role. An ideal candidate will be highly organized, a strong and proactive case manager. The position includes the following essential functions:

Role and Responsibilities

Provide case management services, community health work and resources, information and referrals to community members in the following areas: education and workforce development opportunities; food access including pantries/ food distribution; housing services including landlord/tenants conflicts; identify primary and specialty care, prenatal care, dental care, behavioral health and early intervention services and/or child development; assist families and individuals to completed health insurance application assistance for families and children (new, renewal, and recertification) and SNAP applications in our office in the greater Pottstown area.

  • Develops a comprehensive cost effective service plan utilizing in-house sources, referrals, contracted providers and/or institutions.
  • Conducts on-going monitoring and assessment of service delivery for optimum quality and efficiency, and authorizes and recommends changes and adjustments as needed.
  • Maintains comprehensive case records, summaries, statistical and narrative reports and completes required forms.
  • Refers clients to appropriate resources to assist with meeting goals and advocate for special services as needed.
  • Coordinated all families, children and adults activities including the summer program, ESL classes, art activities, health fairs, health screenings, and other activities.
  • Identify eligible, low-income households in the Pottstown borough in need of lead paint remediation and submit intake documents to process.
  • Record accurate documentation of clients programs and financial files, outreach, referrals, program participation, and other relevant activities in a timely fashion to support monthly reports. Enter data, submit applications and reports in a timely manner.
  • Attend all ACLAMO staff meetings, field trips and trainings
  • Other duties as assigned

 

Qualifications and Education Requirements

  • A Bachelor’s degree with 3 to 5 years of experience in human services. Degrees should be from an accredited college or university in human services, social services and related field of studies.
  • Fluent in Spanish and English required.

Preferred Skills

  • Proficient in Microsoft programs, eg. Outlook, Word, Excel, and Powerpoint
  • Work with a variety of data management using standardized methods and job-related equipment including ability to enter and track client’s data.
  • Comfortable working both independently and as part of a team
  • Excellent written and verbal communication and interpersonal skills.
  • Must be detail oriented and possess strong organizational skills
  • Work at establishing a base of trust with each resident or community member.
  • Accept residents and community members “where they are at” while working to assist them.

  • Observe confidentiality, privacy, and dignity of each person in the program.

Special Notes:

  • Must have and maintain a valid drivers’ license, automobile insurance and daily access to an automobile and cell phone
  • This position requires visual and auditory acuity and the physical ability to ascending or descending ladders, stairs, ramps among others. Adjusting, lifting up, carrying up or moving objects up to 35 pounds in all directions. Standing or sitting for prolonged periods, walk and work in your feet for a minimun of 6 hours at a time.
  • Required working days are Monday thru Fridays and some night and weekends.
  • This position is base in Pottstown but may required traveling to all ACLAMO offices.
  • Must obtain the following three clearances:

Pennsylvania Child Abuse History Clearance;

Pennsylvania State Police Criminal Record Check; and

Federal Bureau of Investigation Criminal Background Check.
