Community Health Worker

Full-time (Norristown)

The community health worker will provide direct services to clients to help them improve their health outcomes. As an important and trusted member of the community, the CHW will provided provide home and office visits. The community health worker will help reduce health disparities and improve long-term health outcomes for at-risk populations for Latinos in both Norristown and Pottstown. She/he will increase access to much needed primary care, behavioral health, and other health services. This position will be a liaison between clients, medical providers, and other local service providers and community groups. The ideal candidate for this position will be highly organized, flexible, kind and patient with the clients. It is also expected that this candidate will be able to communicate effectively with other providers and partners in and out of the medical field.

 

QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • A bachelor’s degree from an accredited college or university in public health, health education or related field with at least 1 year of experience working in these fields.
  • Must be bilingual (English/Spanish).
